FORT COLLINS — Canyon Home Care & Hospice has acquired Columbine Poudre Home Care and , expanding its services across .

The acquisition took effect this past February. Canyon said the deal broadens its ability to provide , hospice, and across the region, including communities from Wellington to Longmont and the Denver area.

Patients will continue receiving care without changes to clinical teams, care plans or scheduled visits. Employees of Columbine Home Care were offered positions with Canyon to maintain continuity of care.

“This isn’t the end of what Columbine Home Care built, it’s the next step,” said Holly Johnson, a former Columbine administrator who joined Canyon. “With Canyon providing hospice care, we can now offer a continuum of care that is needed in northern Colorado.”

In 2025, Columbine Home Care reported more than 10,000 visits, while Canyon reported 49,061 visits.

Rae Roberts, an administrator with Canyon, said the companies had previously referred patients to one another when capacity was limited.

“Now that we’ve joined, our strength is even greater,” Roberts said. “This transition is administrative in nature and allows us to strengthen operations and expand support resources.”
